Why do I have to register?
In order to take the Virtues Assessment Questionnaire and receive your results you must register your name and email address and confirm that you are happy for your data to be collected and stored from the questionnaire.
PLEASE NOTE THAT THERE ARE TWO QUESTIONNAIRES ONE FOR THE ‘SUBJECT’ AND ONE FOR A MENTOR WHO IS EVALUATING THE SUBJECT. PLEASE BE SURE TO SELECT THE CORRECT SUBSCRIPTION AT THE BOTTOM OF THIS FORM.
